I have more information, it's taken form Shiva Pallaki Seva Prabandham (composed by King Shahaji)
the beginning line is " Modamuna Kundala ", Is it a rare book ? I can't find it in any source on the internet.

it is indeed a daru from the pallaki sEva prabandham of Shahaji.. It is notated in the rAga padi. if you have friends in chennai, the carnatic music store has editions of this book, notated by Prof.sambamoorthy, available for some measly 30-40 Rs.

I believe the store also couriers books to buyers..
