HK - Here is my attempt:
upAsmahE - We worship
Ekadantam - the one-tusked Ganesha,
anEkadam - giver of many things,
pArvatIsutam- the son of Parvati,
tam - him
nAkapatinuta varacaraNam - the one whose beautiful feet are worshipped by Indra ( the king of heaven),
pinAkapANi lAlita vadanam - whose face is caressed by Shiva (who holds the bow called Pinaka)
gajAnana vicitrAkAram - the one who has an unusual form with an elephant-face,
agajAnana kamala divAkaram - who is the sun to the lotus that is Parvati's face ( makes it bloom),
gurutarAntarAya shatadhAram - who is like a hundred knife-blades to even great obstacles,
gamitapumartha - who fulfils all the (four-fold) purposes of men (Dharma, Artha , Kama and Moksha),
mOdakadharakaram - who holds the Modaka ( sweet),
grhItaika vimshatyavatAram - who has taken 21 forms,
gIrvANAgrima pUjAdhAram - who claims the first Puja among all the gods,
gAndharvAdi vidyAdhikAram - who is an authority on all branches of learning beginning with music,
gANapatya shrI dEvOddhAram - who is the god of "gAnApatya" faith and who uplifts (its followers)
(gAnApatyam is one of the six faiths (ShaN-matam) established by Adi Sankara. It is the worship of Ganapati as supreme god. The other five are Saivam, vaishNavam, kaumAram, SAktam and sauram)

Rajani: Thanks for the corrections.
I checked in the book and made a one correction in my database (pUjAdhAram) as you indicated and changed gaNapatya to gANapatya (as printed in the book). Also shatadhAram is the way it is in the book.
