Would someone please post the lyrics for the "dwAdasha nAma stOtram" in sanskrit or english? It's Shri Madhvacharya's composition.

Did you mean the one by Ananda tIrtha of the madhva tradition?

Shri Madhvacharya (aka Anandatirtha; aka Poornaprajna) has composed Dwaadasha Stotra in Sanskrit. As pointed out in the previous thread, dvaita.org is one of the best places on the web to understand Shrimadacharya's compositions.

There are a few books available which contain the stotra in Sanskrit as well as English translation/transliteration. The best place to procure in Bangalore would be Poorna Pragna Vidyapeeth - it is located somewhere near Basavanagudi. If you live in Chennai, Ramakrishna Mission in Mylapore has a publication on this subject.
