1: pankajAta bhva vEdyam - who can only be understood by (the likes of) Brahma
hrdyam - who is pleasing to the heart
pankajAta bhavarOga vaidyam Adyam - who is the primary physician in curing the disease of rebirth
2: sadguNa sAndram - who is densely packed with virtues
shrI mahAdEva sarasvatI samyamIndra candram - the king of Sanyasis who has the name of Mahadevendra Sarasvati , who is like the moon (pure and soothing)
(samyami means Sanyasi )

I think "Pankajatha bhava vedhyam" represents one who has understood Lord Brahma and Lord Shiva - Bhava may indicate Lord Shiva here.
pankajapta (2nd) - made out of dirt i.e., material world and physical existence
saandram - repository
I think "sri mahadeva saraswathim" refers the one dedicated to Lord Shiva and Lord Saraswathi.
Madyama Kalam

shankarAgasita - shanka + sita + raga i.e., doubts + tied with + desires (Also may be it denotes Raga Shankhabharanam)
With his lotus hands (kara +abja) he can remove (dhurya literally kill) all the doubts (sankha), desires (raga) tied with (sita) troubles (ayasho) forever (anisham) as he was endowed with irresistible (avaarya) fame for valour (shaure) of his penance (tapas)
Charanam

aalavaala - basin for water round the root of tree.
He is aalavaala around the creeper (lathaa) of divine (parama) wisdom (Jnana).
He traps (jaala) good minds(su + mano) with his humble (taara) behavior (bhavya).
He is expert (sheelam) in uprooting (khandana) the wrong or cruel (chandima) belief (matha) of others (para).
He established (sthapana) the principle of Advaitya just like playing (leelam) a divine (parama) sport.
